My gas fireplace will not light. What are the usual causes?

Most often a pilot that has gone out, a worn thermocouple or thermopile that is no longer sensing the flame, a failed igniter, or simply an interrupted gas supply. Remote batteries and tripped safety switches account for a surprising share too. A gas appliance has several safety components that all have to agree before it will light, so guessing at parts is an expensive way to work through it.
Something is stopping the flue drawing. That can be a closed or seized damper, a blockage such as a nest or leaves, a cold flue that has not been warmed before lighting, or negative pressure in a tightly sealed house pulling against the chimney. Air conditioning and extractor fans make the last one worse, which is why it can appear in a fireplace that used to be fine.
It can be. Refractory panels are what stand between the fire and the structure of your home, and cracks let heat reach places it was never meant to. Fine surface crazing is normal with age. Cracks you can fit a coin into, missing sections, or panels pulling away from the wall are not, and the fireplace should not be used until they are assessed.
For wood: an inspection, a sweep if there is buildup, and a check of the damper, firebox and cap. For gas: the pilot assembly, thermocouple and igniter checked, the burner ports cleared, the logs positioned correctly, and the glass and gasket inspected. Log position matters more than people expect, because logs out of place cause sooting and can block the burner.
Usually, but not without looking first. The common findings are a nest in the flue, a seized damper, water damage to the crown or firebox, and on gas units a pilot assembly that has corroded. Most of it is repairable. What matters is that it is inspected before a first fire, not after.
